Lightbulb Ratchet assembly and shifting issues

I recently had to replace my Ratchet shifter because of not being able to up shift smoothly replaced all parts of the assembly, re-installed everything and it worked great for two weeks. Then started having more issues up shifting.
I removed everything inspected all parts. the Star shift r looks good no scraps or issues. depressing the ASM roller i can cycle the tire and move the shifter smoothly through the gears no binding or tightness 1 N 2 3 4 5 and back down. put it all back together and it drops into first and wont shift up.

I thought if the RAC clutch spun a few times and i did not catch it when i put the clutch pinion back in would that off set the alignment in any way between the ratchet assembly teeth and star shifter?

feels like the shifter assembly is slipping in front of the star shifter.
Does any one have a idea what this could be ?

In and out play in the shifter shaft? Pull the primary cover and put an oil drain plug crush washer over the end of the shift shaft to take up that play.

I have had the Primary cover off 4 times now I will make it 5 and try your fix.
